SimpleApp now uses FlyCamAppState to manage its

camera (by default).  If the app state doesn't
exist then the camera doesn't get created.  Due
to the volume of apps that will expect a working
camera in simpleInit(), it wasn't possible to
do this as a purely separate app state.
